We will use this information to form our teams and purchase shirts and other event items. All medications must be handed to the medical team during drop-off at registration, and the youth will retrieve what they need when it's time to take their medicine. All medications should be in their original bottles with the youth's name on them or in store-bought bottles (e.g., over-the-counter allergy medicine).
 
 
 
 
 
 
In Case of Medical Emergency
I do hereby give permission to the physician selected by the youth ministry leadership to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child as named on the above form. I do hereby release Cornerstone Church of Augusta and its leadership from any liability whatsoever arising out of injury, damage or loss, which may be sustained during his/her involvement with the youth group during all ministry activities and trips:

Event Cost: $100 per student



A $25 deposit is required at registration to reserve your spot. The remaining balance can be paid later.



We encourage students to talk with their youth leader about fundraising opportunities that may help offset the remaining cost.
